Letter of Recommendation - Completed, signed and dated by school official
Official copies of school report cards, in English, for the past two years and current year in .pdf format
Copy of valid passport
Copy of notarized custodianship declaration documents - Parent & Custodian
Photocopy of Birth certificate in English
Copy of immunization record in English
Pay non-refundable application fee
