- Academic Application Form. Completed, signed and dated by both parents and student
- Letter of Recommendation - Completed, signed and dated by school official
- Official copies of school report cards, in English, for the past two years and current year in .pdf format
- Copy of valid passport
- Copy of notarized custodianship declaration documents - Parent & Custodian
- Photocopy of Birth certificate in English
- Copy of immunization record in English
- Pay non-refundable application fee
- Digital photo

- Homestay/Custodianship Application (if applicable) - Completed, signed and dated by both parents and student
